A game Lilian. Episode One: Crazy Friends talks about a girl with extraordinary abilities. Her parents did not like that their daughter was different from other children, so they were committed to a psychiatric hospital. The main character has a teddy bear that she loves very much. His name is Bob and she made him out of different character pieces.

The player takes control of Lillian in the hospital. The user has several tasks to perform while navigating the two-dimensional world using side-scrolling. You have to find a way out of the hospital and explore the surroundings, dark corridors, constantly moving into a parallel dimension. The player will distinguish reality from illusion and try to hide from the nightmares that haunt the protagonist. The real world of Lylian: Episode One: Paranoid Frendship is interwoven with paranoid consciousness and nightmares.

Along the way, the main character will face different enemies and nurses who will have to fight to advance. The passage begins with an escape from the chamber. A mysterious shadow helps the ward and unties the straitjacket. Then Lillian ruthlessly eliminates the Pied Pipers, children like monkeys and nurses. Soon she will face the Lord of the Rodent Squads, a man of enormous stature with a pipe.
